At a minmum, you need a good ISDN Receiver/Transmitter. The Telos Zephyr is pretty much the industry standard. then, of course, you need your local phone company to install the ISDN lines in your home/studio/closet/whatever.
After that, It's a matter of a quiet room, a good microphone and some nice copy to read.
It should be noted that, if you don't have to be in immediate contact with your clients over ISDN, you can always do the session while the client listens in over the phone (There are even systems,like Genter, that allow you to patch your phone into your recording device) - in which case, you can record directly onto your computer, and then mp3 or .wav or aiff the files to your client.
Somebody with more computer savvy than I can probably set you up with that.
